Healthcare for Vulnerable Populations

The collaborative efforts of various programs in Ventura County, including Whole Person Care, Backpack Medicine, Healthcare for the Homeless Clinic (HCH), Interface CandF Services, and Project Understanding, aim to address the multifaceted needs of our community members experiencing homelessness. These initiatives provide essential services such as patient navigation, housing assistance, case management, shelter support, addiction care, HIV treatment, and counseling services. The overarching goal is to connect individuals with primary care, ensuring that they receive comprehensive healthcare, including addiction treatment, transitional support, and assistance in navigating emergency room visits. These programs collectively work towards improving the well-being and overall quality of life for those facing homelessness in our community.

Backpack Medicine is a program led by doctors that aims to provide essential healthcare services to individuals who are homeless. We work to connect them with traditional healthcare settings like clinics. Our program also includes special initiatives such as mobile showers, a mobile medical clinic, addiction medication-assisted treatment, and counseling services, which cover addiction recovery programs and support.

We offer vaccines and health screenings to anyone in need. Additionally, we provide assistance with housing support and placement, access to shelter services, as well as essential items like toiletries, clothing, and meals. Our goal is to improve the health and well-being of those experiencing homelessness and help them transition towards more stable living conditions

Backpack Medicine is made possible through collaboration with various partnering agencies in Ventura County. These agencies include Healthcare for the Homeless/One Stop program, Backpack Medicine Project, Shower Podz, Laundry Trailer, Whole Person Care Program/CalAim, Ventura County Medical Center Family Residency program, Ventura County Public Health (including the Communicable Disease Program, Immunization, and HIV services at The Center), Salvation Army (a California Corporation), Ventura County Human Services Agency (Homeless Services Program, Cal-Works, Cal-Fresh, and General Relief), The Rescue Mission of Ventura County, Turning Point Foundation, Ventura County Behavioral Health (Rise and Path programs), local law enforcement agencies, Project Understand, Harbor House, Lutheran Social Services, Samaritan Center, and Ventura County Continuum of Care.
