Our dedicated team of healthcare professionals offer a range of services tailored to meet the specific needs of our community members experiencing homelessness- from basic medical care and preventative screenings to mental health support, addiction services, social services, shelter, and basic needs.

We strive to meet our patients where they are, both physically and emotionally, by providing a holistic approach to our care and services.

Through partnerships with local agencies, we aim to provide medical treatment and support in navigating the complex challenge of homelessness. We foster a safe and welcoming environment where every patient feels valued, respected, and is treated with dignity and compassion.

We offer the following Programs:

Health Care for the Homeless Clinic Services

Our clinic-based services include health assessments, diagnosis, and treatment for homeless or transitionally homeless individuals in addition to:

  • Designated Sharps Drop-Off Location
  • Opioid Overdose Prevention Education and Narcan Kit distribution
  • VCMC Transitions Care for Patients without an Established PCP or Medical Home
  • Addiction Medicine
    • Conditions We Treat: Opioids, alcohol, stimulants, benzodiazepines, mood enhancers, gambling, sex, eating disorders, technology, among other conditions, in adolescents, adults, pregnancy, and elderly. 
    • Medication Assisted Treatment (MAT)
    • Our physicians are trained to prescribe medications that can help with withdrawal symptoms, cravings, and maintaining sobriety- including (Suboxone/Subutex) and naltrexone (Vivitrol).
    • Counseling Services
      • Counseling is an integral aspect in a successful treatment program.
      • Behavioral Health Clinician (LCSW)- focuses on dual diagnosis.
      • Alcohol Drug and Treatment Specialist (ADTS)- Facilitates individual and group therapy.
    • Continuity of Care
      • Adjust treatment plan to meet individual needs as they progress toward their goals and recovery.
    • Case Management
      • Substance Use Navigators (SUN)- Located in the hospital 7 days a week, provide depression screening, readiness assessment, introduction of the treatment options, and offer warm handoff to our outpatient clinic.
      • Whole Person Care- We work closely with their team to identify individuals who are in the hotels, need referral to our services, facilitate transportation, sober living, and other related services. For more information click here.

Backpack Medicine

This program helps bridge the gap and provide medical, social, behavioral health, and substance use services to individuals living in the local river bottoms, homeless encampments, and parks, linking individuals to long-term programs and services.

The team includes VCMC Residency Program Providers and Faculty, HCH & One Stop Personnel, Whole Person Care, Behavioral Health RISE/PATH, Turning Point Foundation, Public Health Department, and our local Law Enforcement.

Backpack medicine occurs 2x a week- Mondays and Thursdays, times and locations vary on community needs. 

One Stop

Offers a wide range of services for individuals who are experiencing homelessness or at risk for homelessness. Locations include Ventura, Oxnard, Camarillo, and Santa Paula. To see the flyer and location schedule, click here.
