Whole Person Care (Cal-AIM)

What Is CalAIM?

CalAIM, or Whole Person Care, is a long-term commitment to transforming and strengthening Medi-Cal, making the program more equitable, coordinated, and person-centered to help people maximize their health and well-being.

Who Is Eligible?

Gold Coast Health Plan members who are experiencing one of the following may be eligible and are encouraged to request a referral:

  • Homelessness
  • Serious mental illness or substance use disorder
  • Frequent hospital or ER visits
  • Recent incarceration

How To Get a Referral

Enhanced Care Management

Managed Care Plans that already provide medical coverage for low-income Californians, are now able to provide access to other necessary supports to improve the health and well-being of Medi-Cal members including Enhanced Care Management, and Community Support for help with other social services such as housing and food.

In Ventura County, our local Managed Care Plan is the Gold Coast Health Plan (GCHP). Gold Coast Health Plan has contracted with Ventura County Health Care Agency to work together on the implementation, launch, and administration of these services.
