Thank you for your interest in an observation in one of our WIC sites. Each observation is 4 hours from 8:00 am until 12:00 pm or 1:00 pm until 5:00 pm. Activities may vary by site and/or day, but are sure to provide a good overview of WIC Services. Please read (and print if desired) the relevant documents below.
STEP 1: Review the Observation Availability Table below. Identify up to four observations which match your own availability.
STEP 2: Complete the Nursing Student Intake/Approval Form for WIC Observation and return to Amy Avelar - amy.avelar@ventura.org - Instructions are on the form. If your availability changes, please send a replacement form.
STEP 3: Amy will assign an observation to you, complete the bottom of the form, and email the form back to you, your PH preceptor, school contact and the WIC site supervisor. She will include instructions to bring that form as well as a signed Confidentiality Agreement to the observation with you.
