How to Obtain a Birth or Death Certificate

Attention: The following information is intended for the general public. If you are a funeral director and need to order Death Certificates/pay for a Burial Permit, please refer to the section labeled “General Information for Funeral Establishments Only” in the menu on the left.

The fees for certified copies are listed below, and this fee is non-refundable. If a record is not found, a Certificate of No Record will be issued.

To request in person:

  1. Print the application.
  2. Complete sections 1 & 4.
  3. Please bring your completed application, payment, and a valid government-issued ID to our office during regular business hours. Please note our office accepts Visa or Mastercard credit cards and does not accept bills over $20.00.

To request by mail:

  1. Print the application.
  2. Complete sections 1, 2, and 4, and have section 3 notarized.
  3. Mail the completed, notarized application to our office along with a check or money order; please note all payments by check will be on hold for 14 business days.
