Nurse-Family Partnership (NFP) empowers first-time moms to transform their lives and create better futures for themselves and their babies. Over more than four decades, research consistently has proven that Nurse-Family Partnership succeeds at its most important goals: keeping children healthy and safe, and improving the lives of moms and babies.

Services

When a mom enrolls in Nurse-Family Partnership, she is partnered with her own specially trained Public Health Nurse (PHN). She receives regular home visits starting as early as her first trimester and through her child’s second birthday.

NFP nurses support mothers successfully transform their lives, and that of future generations, starting with developing a strong, healthy relationship. The PHN becomes a trusted resource that moms and families can rely on for support and resources on everything from safely caring for their child to taking steps to provide a stable, secure future for them both. Through NFP, the PHN provides new moms with the confidence and the tools they need not only to assure a healthy start for their babies, but to envision a life of stability and opportunities for success for both mom and child.

NFP is a free and voluntary program.

Who is Eligible?

• First-time moms (28 weeks gestation or less)
• Eligible for Medi-Cal or WIC (low-to-moderate income)
• Ventura County resident

How to Enroll in NFP

Anyone can refer to NFP. Women who meet the eligibility requirements can refer themselves (self-referral), or they may be referred by a healthcare provider or community agency. Call 805-981-5115 for more information or complete the referral form.

Following the referral, pregnant women will be screened and assessed for NFP eligibility. If a woman is not eligible for NFP, she will be offered information on other home visiting and supportive services in Ventura County.
