Community Health Nursing

We work to protect and improve the health of individuals, families, and communities by empowering individuals to achieve optimal health and assuring access to health care. We do this by assessing the needs of individuals across their life span, addressing their concerns, identifying their needs, developing health plans and interventions, and achieving their health goals.

We accept referrals from many sources throughout the community, including physicians, social workers, the general public and you.

Based off your needs, circumstance, or preference you will be assigned a public health nurse that will best support you. If you have any questions regarding eligibility or about the referral process, you may call 805-981-5115.

Community Health Nursing Programs

CALWORKS
Works with those CalWORKs recipients participating in the Family Stabilization program.

FIELD NURSING
Works with all ages with a focus on maternal child health. Open to all the community.

 

NURSE-FAMILY PARTNERSHIP
Works with 1st time moms who are under 28 weeks pregnant for the duration of the pregnancy and until the child turns 2 years old.

THRIVE
Works with school age children involved with SARB, probation youth, and CSEC.

 

If you need help with linking to health care services, have concerns about your child’s development or your pregnancy or have other health-related questions, please submit a referral to Community Health Nursing. Any questions regarding the referral can be sent to chnreferrals@ventura.org.
