Chronic Disease Prevention Program
The Ventura County Public Health (VCPH) Chronic Disease Prevention Program (CDPP) offers multiple services focused on the prevention and control of chronic diseases such as prediabetes, type 2 diabetes, high blood pressure, and heart disease, as well as opportunities for physical activity including free Zumba® and Seated Tai Chi classes for adults. Trained health educators and lifestyle coaches provide education, information, and resources to individuals, families, and community groups to equip them with important skills needed to lead healthier lifestyles that are critical in preventing chronic disease and improving health and wellness. Referrals and resources to health care and social assistance programs are provided according to individual needs.
Program Services
CDPP offers a variety of health promotion and disease self-management workshops as well as opportunities for physical activity classes in low-resource communities.

Partnerships and Collaboratives
The Partnership for a Healthy Ventura County, an initiative of Ventura County Public Health, is a coalition of multiple programs and agencies serving diverse populations groups working together to support policies, systems, and environmental changes that promote healthy eating and active lifestyles with the goal of countering obesity and its related chronic diseases.
