Adolescent Family Life Program
Purpose
This program is designed to address and enhance the health, social, economic, and educational well- being of expectant and parenting adolescents/youth and their children in California. The program supports youth in building resilience, strengths, and skills to thrive.
Goals
- Increase social and emotional support
- Improve health birth outcomes
- Improve pregnancy planning and spacing
- Increase educational attainment- School graduation and educational or vocational advancement
- Increase access to and utilization of needed services
Services
Social workers provide comprehensive case management services on a home visitation model to eligible expectant and/or parenting teens/youth. They meet with clients 2x a month. Social workers work predominantly with the low-income, high-risk teen population. Provides education and 12-24 months of free case management services. Our Social workers assess the client’s situation and resource needs while prioritizing personal goals. They assist with staying or returning to school and attaining a positive outcome. This program helps develop effective parenting skills and obtain social/ economic independence by building self -esteem and self-worth.
Who Is Eligible?
Eligible youth must be:
- Expecting and/or Parenting female and male who is 21 years old or younger
- Resides in Ventura County
