We provide a wide range of services that link clients with health care, psychosocial, and other services. This includes treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
Who We Serve
Individuals with HIV/AIDS are eligible for Medical Case Management services with proof of HIV/AIDS diagnosis.
