The Health Care Agency’s Ambulatory Care Department provides a forward-thinking and unique approach to health for our patients with Diabetes. The Las Islas Diabetes Center offers a team approach to treat the patient. Physicians, diabetes educator, dietician, social worker, psychologist and a case manager, all work together to create an individual plan specific to an individual’s needs.
They provide both adult, pediatric and gestational diabetes management, foot and wound care, complex insulin regimens, including insulin pump therapy and glucose monitoring, individualized nutrition therapy, retinal screening and evaluation and a diabetes and healthy copy group therapy class.
