Application for Still Birth Certificates

Authorized Persons: Per the Missing Angels Act, SB 850, you must be the mother or the father of an infant for whom a fetal death certificate was registered in California to request a Certificate of Still Birth. The Certificate of Still Birth must be requested from the County in which the fetal death is registered or from the California Department of Public Health, Office of Vital Records. Certificates of Still Birth may be requested from our office for fetal deaths that occurred from 2008 to the current year.

In Person

Visit the Vital Records Office 9am – 4pm Monday thru Friday to complete an application form. You must present a valid, government issued photo ID at the time of purchase. Because Certificates of Still Birth are completed only as requested it may take up to one business day to process your request.

By Mail

Mail a completed, Certificate of Still Birth application form and a self-addressed, stamped envelope to the Vital Records office. The address is noted at the top of the application form. Mail orders paid by personal check are held for 14 working days. Mail orders submitted with a cashier’s check or money order are processed within two business days of receipt. If no record is located, the fee is kept for the cost of the file search.

Fee: $14.00 per certified copy.
