Public Health Nursing

We work to protect and improve the health of individuals, families and communities by empowering people to achieve optimal health and assuring access to health care. We do this by assessing the needs of individuals and families; identifying how we can help; working with you to create health plans and goals; and, helping you achieve those goals.

 

We accept referrals from many sources throughout the community, including physicians, social workers, the general public and you.

If you need help with linking to health care services, have concerns about your child's development or your pregnancy or have other health-related questions, download the updated Nursing Referral Form (PDF) and email it to the address below or FAX it to 805-654-7610.
